Job summary

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance or access to a dependable vehicle
• Public transportation access acceptable
• Current CPR certification or ability to complete within 90 days of hire

Responsibilities

• Provides high quality clinical services within the scope of practice per plan of care and infection control procedures
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements
• Participates in care coordination, reporting needs and changes in condition, attending case conferences
• Function in home settings regardless of patient demographics
