Qualifications

• Documents patient visits thoroughly and completely per regulatory and payer requirements.
• Actively participates in the agency's care coordination process including timely reporting of patients' needs and changes in condition.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical services within scope of practice as defined by state laws.
• Documents patient visits in the EMR and adheres to infection control procedures.
• Coordinates care with the health care team and case coordinators.
• Communicates effectively with patient, family, physician, and other clinicians.
