Job summary

Pleasant Valley Home Health, part of LHC Group and the Optum family, is hiring an Licensed Practical Nurse to deliver quality nursing care in patients' homes. You will follow the plan of care, coordinate with the health team, and ensure infection control standards while supporting patients in a comfortable home setting.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.
• Home care experience preferred.
• Ability to manage multiple tasks and work independently.
• Strong communication, writing, and organizational skills.

Responsibilities

• Provide high-quality clinical services within the scope of practice, per plan of care, and in coordination with the health care team.
• Document patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Coordinate care, report patient needs and changes in condition, and communicate with family, physician, and team.
• Function in diverse home settings and treat all clients with dignity and respect.
