Job summary

Wellstar Health System, Inc. is seeking a Fulltime LPN for day shifts in home health within the Wellstar territory around Kennestone/Cobb/North Fulton/Paulding. One year of clinical experience in home health or medical-surgical settings is strongly preferred.

The LPN provides direct patient care under the supervision of an RN or manager, delivering age-appropriate interventions and coordinating care. Responsibilities include patient education, documentation, and participation in staff meetings

Qualifications

• High School Diploma or GED is required; nursing certificate may be required.
• BLS certification required; all certifications must be current at hire; LPN licensure required.

Responsibilities

• Provide direct patient care under RN supervision and nurses' directions using the nursing process.
• Perform routine nursing procedures to meet patient needs under RN supervision.
• Coordinate care with RN and other team members.
• Educate patients and caregivers to foster self-care in the home.
• Ensure timely and compliant documentation and adherence to regulatory guidelines.
