Qualifications

• LPN licensure with active status in Georgia.
• 1 year medical/surgical or home health experience preferred.
• Strong communication, organization and time management skills.
• Ability to travel to patients' homes for visits.

Responsibilities

• Direct patient care in home health settings under RN supervision.
• Coordinate services with RN and other health team members.
• Educate patients and families to promote self-care at home.
• Maintain accurate documentation within required timeframes.
