Wellstar Health System is seeking a Licensed Practical Nurse to provide direct patient care in a home health setting. The LPN will coordinate with RNs and multidisciplinary team members, and educate patients and caregivers to support self-care at home.

Responsibilities include documentation, attending staff meetings, and staying current with clinical knowledge under Wellstar Health System policies.

Qualifications

• High School Diploma required.
• LPN license required.
• Basic Life Support (BLS) certification required or instructor level.

Responsabilités

• Direct patient care per physician orders with RN oversight.
• Coordinate care with RN and other team members.
• Educate patients and caregivers for home health needs.
• Accurate documentation within established timeframes.
• Participate in staff education and ongoing professional development.
