Job summary

Wellstar Health System, Inc. is seeking a Full-time LPN for day shifts in the home health field serving Wellstar territories around Kennestone, Cobb, North Fulton, and Paulding campuses.

One year of clinical experience in Home Health or Med-Surg is strongly preferred. The LPN will work under RN or manager supervision, perform direct patient care, participate in education and staff development, and may precept new staff.

Qualifications

• Minimum 1 year medical/surgical nursing in an acute care environment (preferred).
• LPN license is required; BLS certification is required.
• Strong written and verbal communication skills; good organizational abilities.

Responsibilities

• DIRECT PATIENT CARE: Assess and manage patients with physician orders under RN oversight; follow nursing process.
• COORDINATION OF SERVICES: Coordinate care with RN and team; notify supervisor of changes in patient condition.
• PATIENT / CAREGIVER EDUCATION: Educate patients and caregivers; promote self-care.
• COMMUNICATION AND DOCUMENTATION: Complete documentation timely; follow regulatory guidelines; participate in care team conferences.
• STAFF EDUCATION AND PROFESSIONAL DEVELOPMENT: Support orientation and ongoing education of staff; stay current in clinical knowledge.
