Empath Home Health, a member of Empath Health, is seeking a caring full-time Home Health LPN to deliver individualized care in the Naples area of Collier County.

Responsibilities

• Provide nursing services per agency policy and plan of care under RN supervision.
• Observe patients and report adverse reactions to medications or treatments.
• Assist physicians/registered nurses with plan of care and procedures.
• Provide patient and family education on diet, medications, and treatments.
• Collaborate with the interdisciplinary team to ensure holistic care.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in Naples area (Collier County) Schedule: M-F 40 hours; weekend and holiday rotations required.

What You'll Do

Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.

Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.

Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.

Performs patient and family instruction on diet, medication, and treatment.

Collaborate with the interdisciplinary team to ensure seamless, holistic care.

What You'll Need

Active Florida LPN license

Applicants must be a graduate of an approved nursing program

Requires 1-2 years of LPN experience Home health care experience preferred.

Intravenous (IV) Certification strongly preferred.
