Job summary

Freedom at Home is seeking compassionate Licensed Practical Nurses to join our Muskegon, MI team providing in-home care. As a 5-star agency, we support clinicians with work-life balance, competitive hourly pay, and flexible full-time, part-time, or PRN schedules.

Responsibilities include nursing visits under RN supervision, care planning, patient and caregiver education, and timely documentation to ensure high-quality outcomes while patients remain safely at home.

Qualifications

• Graduate of an accredited LPN program.
• Current Ohio LPN license.
• Current CPR certification.
• Minimum of one year of LPN experience.
• Home Health experience preferred but not required.
• Valid driver's license with reliable transportation and auto insurance.

Responsibilities

• Provide skilled nursing care in patients' homes under supervision of a Registered Nurse
• Follow and carry out individualized Plans of Care established by the supervising RN
• Perform skilled nursing visits and complete accurate, timely clinical documentation
• Monitor patient status and promptly communicate changes to the supervising RN and healthcare team
• Educate patients and caregivers on medications, treatments, disease management, and self-care techniques
• Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes
• Participate in case conferences, staff meetings, and quality improvement activities
• Promote safe, compassionate, high-quality patient care while helping patients remain safely at home
