Job summary

Freedom at Home in Michigan is seeking compassionate Registered Nurses to join our home health team. We offer full-time, part-time, and PRN opportunities with excellent pay, paid mileage, and flexible scheduling to support work-life balance.

As an RN, you will perform skilled visits, assessments, and care planning in patients' homes, document OASIS where required, educate caregivers, and coordinate with physicians and therapists.

Qualifications

• Graduate of an accredited Registered Nursing (RN) program.
• Current, unrestricted Ohio Registered Nurse license.
• Current CPR certification.
• Minimum of one year of nursing experience required.
• Home Health experience is preferred but not required.
• OASIS experience is a plus.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.
• Develop, implement, and update individualized Plans of Care.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary team.
• Educate patients and caregivers on disease management, medications, treatments, and safety.
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
• Supervise Home Health Aides in accordance with state and federal regulations.
• Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.
