Job summary

Miami Jewish Health is seeking a Home Care Coordinator to develop and implement homecare services for Florida PACE Center participants. You will coordinate durable medical equipment, nursing and therapy services, and ensure quality care across home settings.

Responsibilities include assessing needs, coordinating services, and participating in multidisciplinary team meetings to improve patient outcomes. Strong nursing background and field visit readiness are essential.

Qualifications

• Graduate of an accredited school of practical nursing preferred or higher level of education.
• One to three years of experience working with frail elderly population and home care (or a combination of experience and education)
• Florida Driver’s License and proof of insurance may be required

Responsibilities

• Assess homecare needs and develop plans of care.
• Assist in safety evaluations for Prospect prior to enrollment and document in EMR.
• Coordinate homecare services including nursing, therapy, and personal care to meet client needs.
• Coordinate and authorize Durable Medical Equipment, consumables, and frozen meals.
• Supervise contracted nursing care for PACE participants in nursing/boarding homes.
• Assess and improve the homecare services system to better meet participant needs.
• Participate in Multidisciplinary Team Meetings.
• Monitor compliance of contracted home care agencies and ALF audits.
• Prepare and coordinate Health Assessment Form 1823 for contracted ALFs.
