Job summary

Miami Jewish Health in Florida is seeking a Nurse Practitioner for the PACE program to provide direct primary medical care and counseling to enrolled participants.

The role requires ARNP or PA licensure, current Florida license, and BLS; ACLS preferred. You will manage acute, chronic, and rehabilitation needs, collaborate with the medical director and interdisciplinary teams, and document care in charts.

Qualifications

• Graduate from an accredited school of nursing.
• ARNP or PA licensure and advanced training.
• Current ARNP/PA license by the Florida State Board of Nursing.
• BLS certification required; DEA license required.
• ACLS certification preferred.

Responsibilities

• Performs comprehensive history and physical on new referrals.
• Manages patients with acute, episodic, and chronic illnesses.
• Admits participants to hospital and coordinates care with Medical Director.
• Provides primary care in nursing home and coordinates discharge planning.
• Participates in family conferences and interdisciplinary team meetings.
• Documents treatment in patient charts.
