Job summary

Miami Jewish Health is seeking a Home Care Coordinator to develop and implement homecare services for Florida PACE Center participants. You will coordinate DME and nursing home care, assess homecare needs, and ensure quality services across multiple therapy disciplines.

The role requires knowledge of English, strong interpersonal skills, and the ability to conduct in-home visits throughout Dade and Broward counties. A valid Florida driver’s license is required.

Qualifications

• Graduate of an accredited school of practical nursing preferred or higher level of education.
• One to three years of experience working with frail elderly population and home care or a combination of experience and education.
• Current LPN license by the Florida State Board of Nursing preferred; BLS certification; valid Florida driver’s license and proof of insurance.

Responsibilities

• Assess homecare needs and develop plans of care.
• Coordinate and implement homecare services (nursing, therapy, personal care) to meet client needs.
• Coordinate Durable Medical Equipment, consumables, and frozen meals.
• Supervise contracted nursing care in nursing homes/board and care facilities.
• Participate in Multidisciplinary Team Meetings and conduct compliance monitoring.
