Job summary

Interim HealthCare of the Twin Cities seeks a casual LPN to visit patients at home across the Twin Cities metro area. You will provide care coordination, disease and medication management, wound care, catheter management, ostomy education, infection prevention, and home safety, with an emphasis on independence and flexibility.

This role requires MN LPN license, CPR, ability to travel, and 2 years of experience; gain experience in a collaborative interdisciplinary team and enjoy benefits like

• Current MN LPN license in good standing.
• 2 years' LPN experience preferred.
• Valid driver's license and reliable transportation.
• CPR certification.

Responsibilities

• Participate in ongoing interdisciplinary assessment of the patient.
• Participate in the development and evaluation of the plan of care with the patient, representative (if any), and caregiver(s).
• Provides services that are ordered by the physician as indicated in the plan of care.
• Recognizes and reports changes in the patient’s condition to the RN supervisor.
• Counsels and educates the patient and family in meeting healthcare related needs.
• Completes timely and accurate clinical notes including addressing patient’s progress.

The casual Licensed Practical Nurse (LPN) - Home Care will enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Hours: Casual - Days

This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the service area is the 11 counties of the Twin Cities Metro Area.

A few must-haves for this position:

• Current license as a Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
• 2 years' experience as an LPN preferred.
• Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
• CPR Certification
