Job summary

Interim HealthCare of the Twin Cities is seeking an LPN to visit patients in their homes in the southwest metro. You will provide care coordination, disease and medication management, wound care, catheter management, ostomy education, infection prevention, and home safety.

This full-time role offers pay of $30.42–$36.98 per hour, mileage reimbursement, generous PTO, 401(k), health insurance, HSA contributions, license reimbursement, CEU support, and scrubs allowance.

Qualifications

• Must hold MN Licensed Practical Nurse license in good standing.
• CPR certification required.
• Valid driver’s license and reliable transportation are required.

Responsibilities

• Participate in ongoing interdisciplinary assessment of the patient.
• Provide physician-ordered services per the plan of care.
• Recognize and report changes in patient condition to the RN.

Home Care Licensed Practical Nurse (LPN):

Hours:

Full-time | Days, Monday - Friday | There will be a weekend and holiday rotation. Work approximately one weekend every 4-6 weeks and 2 holiday's per year. The weekend and holiday rotation is dependent upon staffing levels.

Territory:

This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the general territory for this position would be the southwest metro.

A few must-haves for this position:

• Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
• 2 years experience as an LPN is preferred.
• Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance are required.
• CPR Certification

Some functions of what you would do as an LPN:

• Participatein ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Recognizes and reports changes in the patient’s condition to the RN.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Counsels and educates the patient and family in meeting healthcare related needs.
• Completes timely and accurate clinical notes including addressing patient’s progress.
