Job summary

Benchmark Human Services is seeking an LPN Healthcare Coordinator to provide health care coordination and intervention for individuals in Supportive Living. The nurse will train unlicensed staff to meet health needs and will oversee health records and medication needs.

The role includes advocating for clients’ wellbeing, developing individual health plans, and coordinating care across medical teams, with emphasis on community-based support and routine health monitoring.

Qualifications

• Current Indiana LPN license.
• Experience in Community/Long term care.
• Experience working with individuals with IDD and/or mental illness.

Responsibilities

• Provide healthcare and wellness coordination for individuals in accordance with all state and federal regulations.
• Serve as liaison and advocate for individuals' healthcare with medical and interdisciplinary teams.
• Train unlicensed staff on medication administration, delegated tasks, and monitoring.
• Develop and update health risk plans and health-related policies/procedures.
• Manage transitions of care to/from hospitals, nursing homes, rehab facilities or other agencies.
