Benchmark Human Services is seeking an LPN Healthcare Coordinator to provide health care coordination and intervention for individuals in Supportive Living. The nurse will train unlicensed staff to meet health needs and will oversee health records and medication needs.
The role includes advocating for clients’ wellbeing, developing individual health plans, and coordinating care across medical teams, with emphasis on community-based support and routine health monitoring.

Benchmark is seeking an LPN Healthcare Coordinator to provide healthcare coordination and direct intervention, as needed, for designated individuals in the Supportive Living. The nurse will also provide training for unlicensed staff to meet the healthcare needs of individuals served.
