Job summary

The Avalon of Bloomfield Township is seeking a hospitality focused LPN to join our team in Bloomfield, MI. This role provides direct nursing care to residents and supervises care by Quality of Life Specialists/Resident Assistants.

Responsibilities include ADL support, monitoring resident conditions, updating care plans, reporting to Health Services, and coordinating with other disciplines. Current state-licensed LPN and availability for emergencies are required. EEO Employer.

Qualifications

• Must hold a current nursing license valid in the state worked.
• Willingness to be available for any/all emergencies regarding the community.

Responsibilities

• Assist residents with Activities of Daily Living (ADL) and promote independence.
• Identify resident medical/behavioral changes and help develop care plans.
• Provide daily report to the Director of Health Services on conditions, staffing, and supplies.
• Provide nursing care per physician orders and established standards.
• Ensure preventative, therapeutic, and rehabilitative services and coordinate care with other disciplines.
• Monitor and adjust schedules to address call-outs and staffing needs.
