Aperion Care in Vincennes, Indiana, seeks a Licensed Practical Nurse (LPN) to provide direct nursing care to residents and supervise nursing assistants, ensuring regulatory compliance and a warm, collaborative environment.

The LPN plans daily activities, administers medications, completes documentation, and maintains patient confidentiality while coordinating with physicians and families. Part-time roles with benefits are available.

Qualifications

• LPN with active, unencumbered state license.
• Knowledge of nursing practices and facility regulations.
• Experience in long-term care preferred.
• CPR and First Aid certifications required.
• Excellent written and verbal communication.
• Strong organizational and time-management skills.
• Ability to be flexible with multiple priorities.

Responsibilities

• Perform functions under active license as assigned.
• Provide care based on care plans and physician orders.
• Prepare and administer medications as ordered by the physician.
• Develop and maintain good rapport with inter-departmental personnel and other departments.
• Receive/give nursing report at shift start and end.
• Make rounds with physicians as necessary.
• Assist in activities of daily living and meals.
• Communicate with clinical leadership when needs change.
• Know how to care for residents with dementia.
• Be willing to take on other duties within your knowledge and experience.
