Job summary

Gentle Care Strategies is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting in Vincennes, Indiana. You will administer medications, monitor residents, document care, and supervise nursing assistants.

This role requires an accredited practical nursing program graduate, an active LPN license in Indiana, strong clinical and communication skills, and the ability to lift up to 50 pounds.

Qualifications

Responsibilities

• Provide direct nursing care according to resident care plans.
• Administer medications and treatments safely and accurately.
• Monitor residents and report changes to physicians, families, and nursing leadership.
• Complete nursing notes, physician orders, incident reports, and other required documentation.
• Admit, transfer, and discharge residents as needed.
• Supervise and support nursing assistants during assigned shifts.
• Follow infection-control, safety, confidentiality, and residents' rights requirements.
• Participate in staff training, care planning, and quality-assurance activities.
