Job summary

Alumus is seeking a Licensed Practical Nurse (LPN) to provide in-home nursing care, applying the nursing process to ensure quality and continuity for patients in their residence. You will assess, plan, implement, and evaluate care with a patient-centered approach.

The role requires an active LPN/LVN license, at least 2 years of experience, and ability to educate patients and families on disease processes, medications, and wound care.

Qualifications

• Current, unencumbered LPN/LVN license required or Compact State license- required.
• Minimum of 2 years LPN/LVN experience required.
• Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.

Responsibilities

• Complete and scan required record keeping forms/charts upon the guest’s admission, transfer, and/or discharge.
• Chart nurses’ notes in an informative and descriptive manner that reflects the care provided and the guest’s response to the care.
• Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Director of Nursing.
• Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest’s medical record in accordance with established procedures.
