Job summary

Oak Hill is hiring a Licensed Practical Nurse (LPN) to provide nursing support in a residential group home in Wethersfield, CT. The role involves direct patient care, medication administration, and coordination with an interdisciplinary team to ensure safe, person-centered care.

The ideal candidate will hold an active Connecticut LPN license, with a preference for experience with individuals with disabilities.

Qualifications

• Current Connecticut LPN license in good standing.
• Experience working with individuals with intellectual and developmental disabilities preferred but not required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Commitment to person-centered care, dignity, inclusion, and respect.

Responsibilities

• Provide direct nursing care and ongoing health monitoring for individuals living in a group home setting.
• Administer medications and treatments in accordance with physician orders and applicable regulations.
• Monitor, assess, and document changes in individuals’ health status.
• Coordinate medical appointments, follow-up care, and communication with healthcare providers.
• Maintain accurate and timely clinical documentation and medication administration records.
• Support and educate residential staff on healthcare plans, medication protocols, and individual health needs.
• Respond appropriately to medical concerns and emergencies.
• Promote independence, dignity, choice, and the highest possible quality of life for every individual served.
• Collaborate with an interdisciplinary team to ensure coordinated, person-centered care.
