Job summary

Oak Hill is seeking a Licensed Practical Nurse (LPN) for a full-time overnight role in a residential group home in Wethersfield, CT. The LPN will monitor health, administer medications, and coordinate care with an interdisciplinary team to support individuals with developmental disabilities.

Ideal candidates hold a CT LPN license, have strong assessment and documentation skills, and can work independently within a multidisciplinary team. A $4,000 sign-on bonus is available to eligible applicants.

Qualifications

• Current Connecticut Licensed Practical Nurse (LPN) license in good standing.
• Experience with individuals with intellectual and developmental disabilities is preferred but not required.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license and ability to meet Oak Hill driving requirements.

Responsibilities

• Provide direct nursing care and ongoing health monitoring in a group home.
• Administer medications and treatments per physician orders and regulations.
• Monitor, assess, and document changes in health status of individuals.
• Coordinate medical appointments and follow-up with healthcare providers.
• Maintain accurate clinical documentation and medication records.
• Educate residential staff on healthcare plans and medication protocols.
• Respond to medical concerns and emergencies promptly.
• Promote independence, dignity, and quality of life for individuals.
