Job summary

Oak Hill is seeking a compassionate Licensed Practical Nurse (LPN) to support individuals with intellectual and developmental disabilities in a residential group home in Wethersfield, CT.

This full-time evening position (Mon-Fri, 3 pm–11 pm, 40 hours) offers a $4,000 sign-on bonus, competitive benefits, and opportunities for professional development within Oak Hill.

Requirements include CT LPN license in good standing and ability to work with a multidisciplinary team.

Qualifications

• Current Connecticut Licensed Practical Nurse (LPN) license in good standing.
• Experience working with individuals with intellectual and developmental disabilities is preferred but not required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Valid driver's license and ability to meet Oak Hill’s driving requirements, as applicable to the position.

Responsibilities

• Provide direct nursing care and ongoing health monitoring for individuals living in a group home setting.
• Administer medications and treatments in accordance with physician orders and applicable regulations.
• Monitor, assess, and document changes in individuals' health status.
• Coordinate medical appointments, follow-up care, and communication with healthcare providers.
• Maintain accurate and timely clinical documentation and medication administration records.
• Support and educate residential staff on healthcare plans, medication protocols, and individual health needs.
• Respond appropriately to medical concerns and emergencies.
• Promote independence, dignity, choice, and the highest possible quality of life for every individual served.
• Collaborate with an interdisciplinary team to ensure coordinated, person-centered care.
