Job summary

Lutheran Senior Services is seeking an LPN to provide and coordinate nursing care for residents, following the Nurse Practice Act and maintaining licensure. The role collaborates with the interdisciplinary team to create individualized plans and monitor compliance.

The position includes medication administration, potential IV therapy initiation (IV-certified), and thorough documentation in the EMR. It requires a current LPN license and CPR certification, with preferred gerontological nursing

Qualifications

• LPN licensure in state of practice.
• Must be CPR certified.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practices nursing care within licensure and in accordance with Nurse Practice Act; maintains active license and complies with licensure rules.
• Collaborates with interdisciplinary team to define individualized care plans and ensures regulatory compliance.
• Administers medications and oversees medication ordering and administration; monitors IV fluids when ordered.
• May initiate IV therapy if IV-certified.
• Maintains observation/assessment/documentation per licensure level and care plans.
• Uses EMR to document clinical information per policy.
• Observes resident status and coordinates with other services to ensure continuity of care.
• Maintains safe, functional environment and safety practices.
• Oversees and directs certified staff to ensure EverTrue standards of care.
• Reports incidents per state and federal regulations.
