Qualifications

• LPN licensure required in state of practice.
• CPR certification required.
• Specialized training in gerontological nursing preferred.

Responsibilities

• Practices nursing care within licensure scope and state Nurse Practice Act.
• Collaborates with interdisciplinary team to define individualized care plans.
• Administers medications and monitors IV therapies when ordered.
• Oversees medication ordering and administration as applicable.
• Maintains EMR documentation according to policy and procedures.
• Monitors resident status and coordinates with other services for continuity of care.
• Ensures a safe environment and manages potential hazards.
• Directs and supports certified staff to meet EverTrue standards of care.
• Reports incidents and adheres to regulatory protocols.
