Job summary

Country Meadows Retirement Communities seeks an experienced LPN to provide resident care with autonomy and a team‑oriented approach.

You will engage residents one‑on‑one, assess needs, and coordinate with physicians to deliver continuous, high‑quality care in a home‑like setting. Roles include leadership, shift coverage, and ongoing professional growth within a private, family‑owned environment.

Qualifications

• Active LPN license in the state where employed.
• Minimum of one year experience in assisted living or other health care setting.
• Leadership skills and the ability to form relationships with residents, their loved ones and coworkers.

Responsibilities

• Form bonds with residents, their loved ones and physicians to create care plans using critical thinking and good judgment.
• Coach coworkers to monitor and support residents' needs with urgency and responsiveness.
• Round with physicians, oversee orders and monitor medications to ensure quality care around the clock.
• Educate residents and families about health and wellness issues related to the care plan.
• Serve as and share responsibilities of the Manager on Duty.
