Job summary

Willow Terrace Nursing and Rehabilitation Center in Philadelphia, PA is hiring a Licensed Practical Nurse (LPN) for a full-time role with a $4000 sign-on bonus. You will provide 24-hour resident care, medication administration, wound care, and collaborate with an interdisciplinary team to implement care plans.

The ideal candidate holds PA LPN licensure, with strong communication skills and the ability to manage multiple priorities in a fast-paced setting. Prior experience is preferred.

Qualifications

• Current LPN licensure in the State of Pennsylvania.
• Prior experience preferred.

• Ability to work effectively in a fast-paced environment with multiple priorities.

Responsibilities

• Provide 24-hour care to residents, including vital sign checks, medication administration, and wound care management.
• Conduct assessments and observations to identify changes in resident condition and report to registered nurses and other healthcare professionals.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Offer health education to residents and family members, and facilitate communication with healthcare teams.
• Provide complex medical care, including tracheostomy care, intravenous therapy, enteral and parental feedings, wound care management, and pain management.
• Liaise with healthcare teams to ensure seamless care coordination.
• Focus on a wide range of potential health concerns in long-term care settings, including hospice/palliative care, cardiology, podiatry, physiatry, psychiatry and psychology services.
