Job summary

Wyomissing Health and Rehabilitation Center in Reading, Pennsylvania, is looking for a dedicated Licensed Practical Nurse (LPN) to join our care team on an evening full-time shift. You will administer medications, support activities of daily living, and contribute to individualized resident plans.

Qualifications

• Current LPN licensure in the state of Pennsylvania.
• Prior experience preferred; new graduates welcome to apply.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide direct patient care including administering medications, performing wound care, and assisting with ADLs
• Accurately document patient information, progress, and concerns in a timely and thorough manner
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with interdisciplinary teams to ensure seamless communication and coordination of care
• Maintain a clean and safe environment, adhering to infection control and safety protocols
• Provide emotional support and comfort to residents and families during challenging times
