You will administer medications, monitor vital signs, and assist with activities of daily living as part of an interdisciplinary team.

Qualifications

• LPN licensure in PA
• Prior experience preferred

Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital signs
• Monitor and report changes in resident condition to the nursing staff and physician
• Develop and implement individualized care plans, in collaboration with the interdisciplinary team
• Maintain accurate and detailed records of resident care and services
• Collaborate with other healthcare professionals to ensure holistic care
