Responsibilities

• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Offer health education to residents and family members, and facilitate communication with healthcare teams.
• Provide complex medical care, including tracheostomy care, intravenous therapy, enteral and parental feedings, wound care management, and pain management.
• Liaise with healthcare teams to ensure seamless care coordination.
• Focus on a wide range of potential health concerns in long-term care settings, including hospice/palliative care, cardiology, podiatry, physiatry, psychiatry and psychology services.
