Ark Healthcare and Rehabilitation at Branford Hills is seeking a Licensed Practical Nurse for 16–40 hours per week on the 3:00 pm–11:00 pm shift. The role includes medication administration, patient assessments, admissions coordination, EMR maintenance, and discharge education within a 190-bed skilled nursing facility.

Both part-time and full-time positions are available, with responsibilities coordinating with the nursing leadership and CNAs to ensure compassionate, quality care.

Qualifications

• LPN license in CT required.
• Strong patient-care & communication skills.
• Ability to work independently and in team.

Our facility is seeking a Part - Time, and Full time 3-11 shift, 16 -40 Hours per week, Licensed Practical Nurse that will be responsible for the nursing activities of the wing. Whether working independently or in conjunction with the Director/Assistant Director of Nursing, Registered NurseSupervisor, RN's and CNAs, the LPN will work to ensure the optimal delivery of quality, compassionate care.

Responsibilities

• Medication Administration
• Resident Assessments
• Resident Admissions coordination, including verification of Doctor's orders
• Electronic Medical Records Maintenance
• Administration of treatments and documentation
• Patient education upon discharge for proper maintenance of medications or procedures
• All other duties as assigned by the Director of Nursing Services
