Job summary

Ark Healthcare and Rehabilitation at Branford Hills is seeking a Licensed Practical Nurse to support nursing care on the 3-11 shift, with 16–40 hours per week. The role involves working with the nursing leadership, RNs, CNAs, and coordinating patient care.

Responsibilities include medication administration, assessments, admissions coordination, EMR maintenance, treatments, discharge education, and other duties as assigned. This facility is an EOE.

Qualifications

• Licensed Practical Nurse (LPN) required.
• Experience in skilled nursing is beneficial.
• Shift is 3:00 pm – 11:00 pm; 16–40 hours per week.

Responsibilities

• Medication administration
• Resident assessments
• Resident admissions coordination, including verification of Doctors orders
• Electronic Medical Records maintenance
• Administration of treatments and documentation
• Patient education upon discharge for medications or procedures
• All other duties as assigned by the Director of Nursing Services
