Job summary

The Clare in Chicago, IL is seeking a hospitality-focused Licensed Practical Nurse to join our team. This role provides direct nursing care to residents and supervises care provided by our Quality-of-Life Specialists/Certified Nursing Assistants.

Shifts are 6:30 pm to 7:00 am, full-time with a 3-day week and rotating weekends. We offer competitive hourly pay, strong benefits, and growth opportunities in a community that values life stories and compassionate care.

Qualifications

• Current nursing license valid in Illinois.
• Willingness to be available for any emergencies regarding the community.

Responsibilities

• Assist residents with ADLs and promote independence of daily living.
• Identify medical/behavioral changes and develop or adjust care plans.
• Provide daily reports to Health Services leadership on resident conditions and needs.
• Provide nursing care per physician orders and established policies.
• Coordinate with other disciplines to ensure comprehensive nursing care.

The Clare is recruiting for a hospitality focused Licensed Practical Nurse to join our team! This position provides direct nursing care to residents and delegates and supervises the care provided by our Quality-of-Life Specialists/Certified Nursing Assistants.

Here are a few of the daily responsibilities of an LPN:

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
