Job summary

Great Lakes Management Co. is seeking an LPN to join our senior living team in Minnesota. This role includes monitoring residents, coordinating care plans, and supervising medications to ensure high-quality resident care.

The ideal candidate holds an MN LPN license, has home health or long-term care experience, and demonstrates strong organizational and compassionate skills to support residents and families.

Qualifications

• Maintains a current state license as an LPN (Licensed Practical Nurse) per state regulations.
• A minimum of one year's experience in home health, assisted living or long-term care is preferred.
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision.
• Exhibits a caring heart with a passion to care for the elderly.

Responsibilities

• Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Assures residents have up-to-date care plans per state regulations.
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate.
• Works with manager to assure emergency procedures are in place
