Job summary

Great Lakes Management Co. is seeking an LPN for a part-time role in Bloomington, MN. The LPN will support seniors in an assisted living setting, ensuring quality care and adherence to state regulations.

The schedule is three days per week from 7:30am to 3:30pm, with flexible hours possible. The role requires MN LPN certification, with a focus on patient safety, care plan maintenance, and effective communication with physicians, families, and the care team.

Qualifications

• Must hold current MN LPN license.
• Minimum of 1 year experience in home health, assisted living, or long-term care preferred.
• Strong organizational skills with initiative and minimal supervision.
• Demonstrates caring and compassionate approach to elderly residents.

Responsibilities

• Ensure residents meet admission criteria, monitor status, and recommend placement if needed.
• Maintain up-to-date care plans per state regulations.
• Monitor resident charts and progress notes.
• Communicate changes in resident needs to physicians, staff and family.
• Coordinate resident services with other providers.

LPN R esponsibilities:

• Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN .
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate .
• Works with manager to assure emergency procedures are in place

LPN Qualifications :

• Maintains a current state license as a n LPN ( Licensed Practical Nurse ) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
