Job summary

Piedmont Health Services, Inc. is seeking a Clinic Nurse (LPN) for the PACE Nursing team in Pittsboro, NC. The role supports a comprehensive, patient-centered care model under the PACE program, with a Monday–Friday schedule from 8:00am to 5:00pm.

The position requires an active NC LPN license, BCLS, and at least 1 year of LPN experience, preferably in ambulatory care. Benefits include health, retirement plans, holidays, and CME opportunities.

Qualifications

• Education: Practical Nursing Accredited Program.
• License: Licensed Practical Nurse (LPN) in NC; BCLS.
• Immunizations up-to-date prior to employment.

Responsibilities

• Uses the nursing process: assessment, planning, implementation, evaluation, and recording of patient care and education.
• Coordinates services with other health-care team members to provide complex patient care.
• Provides on-site and telephone triage; answers calls and messages.
• Assists with check-in and participates in interdisciplinary huddles.
• Practices within the NC Nurse Practice Act and Piedmont Health Services policies.
