Job summary

Hospice Savannah Inc is seeking an LPN to provide nursing care to patients and families in home care settings under the supervision of an RN. The role requires compassion, strong communication, and reliable transportation.

The LPN will document patient encounters digitally, maintain confidentiality, and adhere to hospice standards. A two-year hospice/med-surg/home health background, CPR cert, and GA LPN license are required. Flexibility for days/evenings/weekends is expected.

Qualifications

• Graduate of an accredited school of Nursing, with a current GA LPN License.
• Minimum of two years of experience in hospice, oncology, med-surg or home health providing direct patient care required.
• Current CPR certification, valid driver’s license, automobile insurance and reliable transportation required.
• Excellent communication skills (written and verbal) and the ability to interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large along with excellent customer service and documentation skills required.
• Computer proficiency in Microsoft Office products including Outlook, and database knowledge required.

Responsibilities

• Provide nursing care to patients and families under RN supervision.
• Accurately document patient/family encounters using a computer.
• Travel to patient homes using personal vehicle and participate in flexible scheduling as needed.
