Job summary

Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, seeks an LPN for its clinical float pool across multiple sites in Nashua and surrounding areas. You will provide patient-centered care and support across in-person, telephone, and telehealth settings.

You will assess, educate, and administer medications while collaborating with RNs, APRNs, physicians, and other health professionals to ensure safe, high-quality care.

Qualifications

• LPN licensed in NH or compact state.
• Current BLS/CPR certification.
• Graduate of an accredited nursing program or enrolled in an RN program with NH LPN licensure.

Responsibilities

• Provide patient-centered nursing care under supervision.
• Conduct nursing assessments and implement care plans.
• Administer medications, vaccines, and document care in the medical record.
• Participate in telehealth, triage calls, and care coordination.
• Collaborate with the interdisciplinary team to ensure quality care.
