Foundation Medical Partners is the second largest multi-specialty group in New Hampshire, affiliated with Southern NH Medical Center, seeking an LPN to float across family practices in Nashua. The role supports care coordination and patient-centered nursing across in-person, telephone, and telehealth settings.

The eligible candidate will have an NH LPN license, be trained in medication administration and patient education, and work under supervision to deliver high-quality care.

Qualifications

• Licensed Practical Nurse with NH or compact state license.
• Provide patient-centered nursing care under RN/Provider supervision.
• Engage in education, medication administration, and documentation.

Responsabilités

• Participate in nursing process across in-person, telephone, and telehealth.
• Design and implement care plans with interdisciplinary team.
• Educate patients on self-management, medications, nutrition, and wellness.
• Document nursing process accurately in medical records.
• Administer medications, vaccines, and perform point-of-care tests.
