Job summary

Foundation Medical Partners in Nashua, NH seeks a Licensed Practical Nurse (LPN) for its outpatient float pool. The role supports clinical operations across multiple family practices, providing care in-person, by telephone, and via telehealth.

You will participate in nursing assessments, plan of care with the team, educate patients, administer medications, and document care in the chart. NH licensure, BLS/CPR, and program completion are required for this clinical float position.

Qualifications

• Licensed Practical Nurse with NH license (or compact state) in good standing.
• Current BLS/CPR certification.
• Graduate from an accredited nursing program or actively pursuing licensure as RN with NH LPN requirement.

Responsibilities

• Perform nursing assessments and interventions under RN/Provider supervision.
• Design and implement plans of care with interdisciplinary team.
• Educate patients on self-management, medications, nutrition, and wellness.
• Document nursing processes in the medical record with accuracy.
• Administer prescribed medications, vaccines, and treatments; perform point‑of‑care testing.
• Collect specimens as allowed by state Nurse Practice Act.
