Job summary

Foundation Medical Partners in Nashua, NH is seeking a Licensed Practical Nurse for its clinical float pool. This role provides patient-centered care across multiple family practice sites, including in-person, telephone, and telehealth.

Under supervision, you will perform assessments, administer medications, educate patients, and document care. NH license as LPN required; BLS/CPR certification preferred; RN pathway may be pursued while maintaining NH LPN licensure.

Qualifications

• Licensed Practical Nurse with NH or compact state license.
• Current BLS/CPR certification.
• Graduate from accredited nursing school or enrolled in RN program with NH LPN licensure.

Responsibilities

• Perform nursing assessments, interventions, and evaluation via in-person, telephone, and telehealth.
• Collaborate with interdisciplinary team to design and implement plan of care.
• Educate patients on self-management, medications, nutrition and wellness.
• Document nursing process accurately in the medical record.
• Administer medications, vaccines, and perform point-of-care testing.
