Job summary

Qualifications

• LPN license in the state where practicing is required.
• BLS for Healthcare Providers certification required or obtained within 45 days.
• High school diploma or equivalent; GED accepted.
• 1+ year LPN experience preferred.

Responsibilities

• Greet patients and assist with transition into care.
• Record vital signs, medical history, and health screenings.
• Prepare patients for exams and assist providers.
• Administer medications and vaccines as directed.
• Perform lab tasks including specimen collection and point-of-care testing.
• Document care in the electronic health record.
• Collaborate with physicians and team to deliver coordinated care.
