Qualifications

• Licensed Practical Nurse (LPN) in New Jersey is required.
• Strong planning, communication, documentation, and team skills are essential.
• Experience with electronic medical records and MS Office applications is preferred.

Responsibilities

• Perform nursing assessments and implement the care plan for individuals.
• Administer medications and treatments per physician orders and policy.
• Coordinate with the IDT to ensure continuity of care and timely documentation.
• Monitor vital signs, weight, and health status; escalate changes as needed.
• Provide education to staff on health care needs and procedures.
