Job summary

CareDiem Home Health seeks compassionate RNs to provide skilled home health nursing to patients in LOVES PARK, IL. You will deliver care under physician orders, perform assessments, med administration, wound care, and educate patients and families.

This role requires an Illinois RN license, 1+ year of nursing experience (home health preferred), CPR (BLS), reliable transportation, and strong clinical judgment.

Qualifications

• Illinois RN license and CPR certification (BLS) required.
• 1+ year nursing experience; home health preferred.
• Reliable transportation, valid driver’s license, and auto insurance.
• Strong clinical judgment, communication, and ability to work independently.

Responsibilities

• Deliver skilled nursing care under a physician’s order.
• Complete assessments, medication administration, and wound care.
• Monitor and document patient progress using Axxess.
• Educate patients and families on health conditions and care.
• Communicate with physicians and the care team.
• Travel to patient homes.
