Job summary

Empath Health is seeking a Licensed Practical Nurse (LPN) to support hospice patients in the Manatee County and North Sarasota County area.

Working under an RN case manager, you will provide bedside clinical observations, prescribed care, and both physical and emotional support to patients as outlined in the plan of care, while reporting changes to the care team.

Qualifications

• Applicant must be a graduate of an approved nursing program and hold a current Florida LPN license.
• Minimum one year experience in an acute care setting with direct patient care; hospice LPN experience preferred.

Responsibilities

• Under the direction of the RN case manager, provides continuous bedside clinical observations, prescribed care, and physical and emotional support to patients as directed in the plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• The LPN utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life for the patient/family unit.
