Job summary

Cornerstone Management in Minnesota is seeking an LPN to support a resident-centered assisted living program, working every other weekend on 12-hour shifts. The LPN will assist the RN with planning, coordinating, and managing service delivery within regulatory guidelines, guiding overall direction and monitoring of care in the community.

The ideal candidate is MN-licensed, with at least 3 years of experience and a strong aptitude for elder care.

Qualifications

• Must be licensed in MN as an LPN and maintain current licensure.
• Must have a valid driver's license if driving is required.
• At least 3 years of experience in a related field.
• Possess sensitivity and aptitude for working with the elderly.
• Ability to communicate effectively with residents, families and RN Supervisor.
• Possess leadership/supervisory skills.
• Able to prioritize and organize work effectively and efficiently.
• Able to work irregular hours, occasional weekends and evenings.

Responsibilities

• Provide nursing support in accordance with Assisted Living regulations and community standards.
• Monitor resident health, update care plans, and coordinate with physicians/pharmacists.
• Assist with medication management: fill pillboxes, monitor renewals, and coordinate with pharmacies and physicians.
• Communicate and document changes in resident condition clearly and promptly.
• Maintain accurate and up-to-date resident records and nursing notes.
• Support regulatory compliance efforts; assist in preparation for state licensing surveys.
• Ensure privacy and confidentiality of resident information and living environments.
• Work closely with RNs and care staff to deliver high-quality, resident-centered care.
• Share updates through staff communication tools and participate in team meetings.
• Maintain open, respectful communication with residents, families, and team members.
• Carry a cell phone and be available for emergencies or staffing updates.
• Respond to resident concerns with professionalism and compassion.
• Support events, trainings, and family functions as needed.
• Uphold safety standards and report incidents or hazards promptly.
• Foster a positive, team-oriented work environment and represent Cornerstone Culture.

Job Summary: Every Other Weekend - 12 Hour Shifts

This position will ensure that the health and safety of all residents are met. The LPN will assist the RN with planning, coordinating, and managing the assisted living services and act within the Assisted living community regulations. The LPN is responsible for the overall direction, coordination, and monitoring of assisted living service delivery under the direction of the RN.

  • Share key updates through communication tools (e.g., staff communication book).
