Job summary

Ellen Memorial Rehabilitation and Healthcare Center LLC in Honesdale, PA is hiring an LPN Licensed Practical Nurse to deliver high-quality patient care under RN supervision. Qualifications

Current LPN license in the state of Pennsylvania.

Responsibilities

Participate in the delivery of quality patient care, under the supervision of the Registered Nurse, to a diverse population of patients
Administer medications, treatments, and therapies as ordered by the healthcare provider
Conduct vital signs, perform basic wound care, and assist with mobility and ambulation
Develop and implement individualized care plans, in collaboration with the interdisciplinary team
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Assist with daily living activities, such as bathing, dressing, and grooming, as needed
Participate in quality improvement initiatives to ensure optimal patient outcomes
