Job summary

Qualifications

• Active LPN license in Virginia.
• Valid BLS/ CPR certification.
• Ability to care for multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with accepted standards of nursing practice and the plan of care.
• Follow and implement individualized care plans developed with the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per the plan of care and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician.
• Maintain accurate, timely documentation per physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.
