Job summary

Qualifications

• Must be graduate of an accredited LPN program.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Six months of long-term care experience preferred.

Responsibilities

• Provides direct care under RN supervision and follows scope of practice.
• Participates in care planning, rounds with the team, and documents resident status.
• Administers medications and treatments as ordered and maintains narcotic records.
• Transcribes and implements physician orders and communicates changes to the team.
• Executes medication management and safety within regulatory guidelines.
• Completes admission paperwork and supports discharge processes.
• Ensures punctuality and regular attendance for shifts.
